I was just wondering, by what mechanism is it that a disease can end someone’s life, not just make them sick?

Not thinking of anything in particular, but if I had to choose a handful (ones that have been very deadly in the past), I’d choose Black Plague, Smallpox, and dysentery. How do diseases and general and those ones specifically end the life of the afflicted person?

Like what’s a very easily spread, hard to fight, deadly disease? I was just reading a question about the Black Plague, which got me thinking, but even that (which was a huge problem hundreds of years ago) wouldn’t be very hard to fight with today’s medicines. Any ideas?
